Filling in a formThe assessment process

If you look after someone you can request a carer's assessment from your social services department. It doesn't matter if the person you care for currently receives help or not.

The person you look after can ask for a community care
re-assessment or review, if their needs have changed and they need more help.

Getting an assessment

The telephone number and address of the social services department will be listed in the phone book under the name of your local authority. You can ask your GP or district nurse to contact social services on your behalf.

A local authority cannot refuse to carry out an assessment. The person carrying out the assessment will discuss with you what support the person you are caring for needs.
