Woman with zimmer frameWhat is community care?

Local social services departments are responsible for arranging support for older, ill and disabled people.

The person you care for can ask social services for a 'community care assessment' of the services they need.

As a carer, you can also ask for an assessment of your needs. The assessments are based on a discussion with the person needing care, their relatives and other carers and, if necessary, information from other relevant bodies, such as the health or housing authority.

Based on the assessment, social services can provide:

  • help and support directly to you; or
  • help directly to the person you are looking after.
