Going home from hospital can be difficult, and arrangements can be made to help you, depending on your needs.

The arrangements that are made will depend on your needs. You may just need some basic help – perhaps advice on your medication and a letter to your doctor. Or you may need help with practical things such as arranging transport and preparing your home for your return. Information on this type of practical help is given.

Some people need a lot of support and assistance to help them manage at home; the next section looks at how this type of intensive support can be arranged.
